COVID-19 Tests
- Centennial Health is currently preforming COVID-19 Rapid Tests.
- All locations except Broken Arrow are able to perform COVID-19 testing.

COVID-19 Vaccinations:
- Centennial Health has not received COVID-19 Vaccination Supply to date, we are currently working with state and local officials on obtaining COVID-19 Vaccination supply as quickly as possible. Anticipated supply will be obtained in Phase 2 of Oklahoma’s Phased vaccine roll out (See Oklahoma State Phase Schedule Below).

Oklahoma COVID-19 Vaccinations Phase Schedule (AS OF 01/06/2021):
- Oklahoma has adopted a multi Phased Schedule for distribution of the COVID-19 Vaccination.
- Phase 1
- When: Currently in this Phase
- Who: Long Term Care residents / Health care workers providing direct inpatient care / Public health staff conducting front line COVID-19 pandemic mitigation and control
- Phase 1
- Phase 2
- When: Anticipated start is January 2021, however this is subject to change based on federal vaccine supply schedules
- Who: Adults age 65 and older, and adults of any age with comorbidities / Teachers and staff in Pre-K-12 schools and educational settings / Health care workers providing direct, COVID outpatient care and services
- Phase 2
- Phase 3
- When: To be determined when future federal vaccine production outlays are known
- Who: Teachers, students, residents and administrative staff in educational settings outside Pre-K-12 / Critical infrastructure personnel ("essential business/industry" personnel)

- Who: Open to all Oklahoma residents.
